The Alzheimer’s Association, Central & North Florida Chapter serves 31 counties

with education and support as well as raising funds for research.  Support

groups are designed to provide emotional, educational and social support for

caregivers.  Attendees develop coping methods, encourage self-care, learn

regularly scheduled and have an open format that run from 1 to 2 hours

depending on the size of the group and are small with 4-12 members each.

The YMCA’s Diabetes Prevention Program is part of the CDC-led National

Diabetes Prevention Program.  This program helps those who have been

diagnosed with prediabetes, or believe they may be at high risk of developing

type 2 diabetes, adopt and maintain healthy lifestyles by eating healthier,

increasing physical activity, and losing a modest weight of amount in order to

reduce their chances of developing the disease.


The program is open to all individuals regardless of their insurance status.

ElderSource is a private, not for profit agency designated by the state as the

Area Agency on Aging and Disability Resource Center for Northeast Florida.

ElderSource serves as the focal point to which elders, adults with disabilities,

caregivers, and the general public can receive information, referral, assistance

and answers on all aging issues.


Care Transitions Coaching


ElderSource currently has a partnership with UF Health offering patients and

families a Care Transitions Coach.  The purpose is to empower patients and

hopefully not have them to return to the hospital unnecessarily.


Your Coach will visit you in the hospital and explain the program to you.  You will

be visited at home at which time your Coach will:


• Review medications


• Talk with you about what symptoms to look for and what to do if your condition

  gets worse


• Work with you on how to communicate with your doctor’s office


• Give you a Personal Health Record for you to fill out to keep track of  our

  medical appointments, making your care go more smoothly and be less confusing


• Your Coach will call several times to check on you and will work with you for 30 days

SHINE is a program of the Department of Elder Affairs and is operated in this

area through ElderSource.  SHINE offers seniors, adults with disabilities, and

This department is a resource center that offers veterans, their families, and the

community veteran’s entitlement services for eligible veterans and their families.

Eligible entitlements and/or benefits are not automatically provided.  You must

apply for them.


County Veteran Service Officers (VSOs) are certified, skilled professionals

trained in developing veteran’s claims for benefits by reviewing and applying

relevant legislation, current law, protocols and pertinent medical evidence.

VSOs function as attorneys-in-fact for veterans and families they represent.

VSOs are accredited through several veteran service organizations.


The work of service officers involves important matters like helping a veteran get

outpatient treatment or gain admission for inpatient care at a VA medical center.

They assist veterans and their families in filing for VA disability compensation,

rehabilitation and education programs, pension and death benefits, and many

other programs.


This office:


• Identifies benefits that pertain to veterans and their families


• Provides veterans and families with free expert advice regarding their claim


• Provides counseling and information on additional resources


• Completes and files paperwork for benefits from all levels of eligibility


• Advocates on veterans and families behalf through the claims process if necessary

This program provides qualifying veterans the opportunity to hire workers and

vendors to help with their daily care needs, such as bathing, dressing,

housecleaning and cooking.  Workers may be your family members, neighbors

or friends.  Veterans enrolled in this program can make purchases and receive

services to help maintain their independence, thereby avoiding the need to go to

a nursing home or other long-term care facility.
